CCSP – Community Care Services Program
A program that provides community-based long-term care services to help individuals remain in their homes rather than moving into institutional care facilities.

SOURCE – Service Options Using Resources in a Community Environment
An integrated care program combining primary care, case management, and community support for eligible participants who need long-term community-based care.

NOW/COMP – New Options Waiver / Comprehensive Supports Waiver
Waiver programs designed for individuals, including children, with developmental disabilities. These programs offer home- and community-based services as alternatives to institutional care.

GAPP – Georgia Pediatric Program
A Medicaid-based program that provides in-home skilled nursing and personal care for medically fragile children under age 21. (Primarily used in Georgia but referenced in multi-state systems.)

ICWP – Independent Care Waiver Program
A program serving adults with traumatic brain injuries or physical disabilities, enabling them to live independently in community settings instead of institutional care.

EDWP – Elderly and Disabled Waiver Program
Provides home and community-based services that support elderly adults or individuals with physical disabilities, helping them maintain independence and avoid nursing home placement.

SFC – Structured Family Care
A care model in which a family member or live-in caregiver provides daily support and receives compensation and professional oversight.

PCA – Personal Care Assistant
A trained caregiver who assists individuals with activities of daily living (ADLs), such as bathing, dressing, meal preparation, and mobility.

IHSS – In-Home Support Services
A service model allowing elderly or disabled individuals to receive assistance with personal care and household tasks in their own homes.

CHHA – Certified Home Health Aide
A certified professional who provides personal and limited medical care in the home setting under the supervision of a nurse or healthcare professional.

HCBS – Home and Community-Based Services
A general category of Medicaid waiver programs that deliver supportive services in home and community settings as alternatives to institutional care.

CCP – Community Care Program
A term used for programs offering coordinated community-based care and support to help individuals remain safely in their homes.

Medicaid Payor Source – Georgia Waiver and Care Programs

Attend Home Care accepts Medicaid clients under several Georgia waiver programs, including CCSP (Community Care Services Program), SOURCE (Service Options Using Resources in a Community Environment), NOW/COMP (New Options Waiver and Comprehensive Supports Waiver), ICWP (Independent Care Waiver Program), and EDWP (Elderly and Disabled Waiver Program).

Under Georgia’s waiver programs, spouses can serve as caregivers through the Family Assistance program. Eligibility and verification are completed through the GAMMIS system (Georgia Medicaid Management Information System).

Each waiver has specific requirements. CCSP requires an approved Service Plan before care begins. SOURCE participants must be enrolled in the Family Assistance Program. For NOW/COMP, individuals under 18 years old must instead go through GAPP (Georgia Pediatric Program). ICWP supports adults with traumatic brain injuries and physical disabilities. EDWP is designed for individuals aged 60 years or older.

Eligibility checks for Georgia Medicaid services should be performed through the GAMMIS portal.

Attend Home Care provides Medicaid waiver services in the following 27 Georgia counties: Appling, Bacon, Brantley, Bryan, Bulloch, Candler, Camden, Chatham, Coffee, Dodge, Effingham, Emanuel, Evans, Glynn, Liberty (Hinesville), Long, Montgomery, McIntosh, Pierce, Tattnall, Telfair, Toombs, Treutlen, Wayne, Ware, and Wheeler.

In addition to Medicaid, Attend Home Care also accepts VA (Veterans Affairs) clients and Private Pay clients.

Medicaid Payor Source – Eastern Pennsylvania In-Home Care

Attend Home Care generally provides in-home care services covered by Medicaid in Eastern Pennsylvania. While most services require Medicaid, certain county-specific waivers may be available that can cover care even without Medicaid, depending on eligibility. County offices can advise clients on which waivers they qualify for.

In Pennsylvania, in-home care does not require any specific certification. However, there are restrictions on who can serve as a caregiver. A Power of Attorney (POA) cannot be the caregiver, spouses cannot serve as caregivers, individuals financially connected to the consumer cannot serve as caregivers, and for clients under 18 years old, parents cannot serve as caregivers. Veterans Affairs (VA) clients may also be eligible, but approval is currently pending and should be verified with the director.

Waivers available in Eastern Pennsylvania include the OBRA Waiver, ACT 150, and Community HealthChoices Providers. These waivers generally serve adults only.

Medicaid Payor Source – New Jersey In-Home Care

Attend Home Care provides in-home care services for Medicaid clients across New Jersey. Accepted Medicaid plans include Horizon NJ, Aetna BH, Wellpoint, Fidelis, and UHC Community Plan. Eligibility and service verification can be performed through state portals such as NJMMIS, HHAeXchange, Availity, and eFax.

In New Jersey, caregiver restrictions vary by program. Spouses and parents generally cannot serve as caregivers for certain Medicaid services, while individuals with Intellectual and Developmental Disabilities (IDD) of all ages are eligible for appropriate services. The state also allows Personal Care Assistance (PCA), which is a state plan service available to all Medicaid-eligible individuals. PCA does not require enrollment in a waiver program and provides support with daily living activities for eligible clients.

Medicaid Payor Source – Ohio In-Home Care

Attend Home Care provides Medicaid-covered in-home care services across Ohio. Accepted plans include Aetna BH Waiver and Molina Waiver. Eligibility and service verification are conducted through the state’s MITS portal.

Ohio Medicaid services generally require caregiver certification. Accepted caregiver types include Certified Home Health Aides (CHHA), Certified Nursing Assistants (CNA), and State Tested Nursing Assistants (STNA). All caregivers must also have current HHA certification, CPR training, and TB testing; these requirements are typically payroll-deducted.

Waivers available in Ohio include the PASSPORT Waiver, State Plan PCA (Personal Care Assistance), and DODD programs such as IO, Level One, and SELF. Personal Care Assistance (PCA) is a state plan service offered to all Medicaid-eligible individuals and does not require enrollment in a specific waiver program.

Attend Home Care provides services in the following Ohio counties: Butler, Clermont, and Hamilton.

Medicaid Payor Source – Illinois In-Home Care

Attend Home Care provides Medicaid-covered in-home care services throughout Illinois. General Medicaid services in the state do not require caregiver certification, though some program-specific training may be required. Eligibility verification can be performed through ECCPIS; however, access to this system requires training, and staff must complete it to obtain authorization.

Illinois offers several waiver and community-based programs. For clients aged 60 and older, the CCP (Community Care Program) and HCBS (Home and Community-Based Services) provide coverage. For clients aged 18 to 59, Attend Home Care can assist with alternative programs. Spouses and parents can serve as caregivers, but they will typically receive limited hours because they reside in the same home and are responsible for regular household duties. Individuals with Intellectual and Developmental Disabilities (IDD) of all ages may also be eligible, though acceptance depends on the individual’s age.

Medicaid Payor Source – Colorado In-Home Care

Attend Home Care provides Medicaid-covered in-home care services throughout Colorado. Long-term Medicaid is accepted, and most services do not require caregiver certification. Eligibility verification is coordinated through local county human services offices and the state’s HCBS Case Management system.

Colorado offers several waiver and community-based programs. Waivers include EDB and IHSS (Now licensed Family Caregiver Program). Personal eligibility requirements include passing a background check. Spouses and parents can serve as caregivers, and Power of Attorney (POA) representatives can also receive assistance for the client. Individuals with Intellectual and Developmental Disabilities (IDD) of all ages may be eligible, and services are generally available to adults 18 years and older.

Attend Home Care provides services in the following Colorado counties: Adams County (Denver, Rocky Mountain Human Services), Arapahoe County (Developmental Pathways), Douglas County (Developmental Pathways), El Paso County (TRE), Jefferson County (County DHS), Pueblo County (TRE), and Teller County (TRE).

Medicaid Payor Source – Wisconsin In-Home Care

Attend Home Care provides Medicaid-covered in-home care services throughout Wisconsin. Accepted Medicaid plans include Forward Health, Community Care – CMO, and IRIS (Include, Respect, I Self-Direct). Eligibility and service verification are conducted through the WIMMIS Forward Health Portal.

In Wisconsin, caregivers do not need certification as a CNA or in any other professional category. Waivers and programs available include IRIS (a self-directed HCBS waiver), State Plan PCA (Personal Care Assistance), Family Care (HCBS Waiver), and Family Care Partnership/PACE. Personal Care Assistance (PCA) is a state plan service available to all Medicaid-eligible individuals and does not require enrollment in a specific waiver program.

Attend Home Care provides services in Kenosha and Racine counties. The organization also accepts VA (Veterans Affairs) clients and Private Pay clients.

Medicaid Payor Source – Kentucky In-Home Care

Attend Home Care provides Medicaid-covered in-home care services throughout Kentucky. General Medicaid services are only accepted through specific waivers, including the SCL (Support for Community Living) Waiver and the Michelle P. Waiver for individuals with Intellectual and Developmental Disabilities (IDD), for both adults and children. Eligibility verification is conducted through the KYMMIS portal.

In Kentucky, spouses and parents generally cannot serve as caregivers under standard Medicaid services. However, under the SCL and Michelle P. Waivers, spouses and parents can serve as caregivers, though they typically receive limited hours because they live in the same home and are responsible for regular household duties. Individuals with IDD of all ages are eligible for these waivers.
